**About The Role**:
The Dorset Council Adults and Housing Directorate delivers adult social care, housing, and a range of community services to people of all ages living in Dorset within the context of the Dorset Council Plan.

The Council Plan is focused on making Dorset ‘A great place to live, work and visit’. This will be delivered through 5 priority outcome areas: Staying Safe and Well, Economic Growth, Unique Environment, Suitable Housing, and Strong and Healthy Communities. The Directorate contributes to these corporate outcomes.

The Adults and Housing Directorate has launched an ambitious transformation programme to ensure Adults, Housing and Community Services are strengths based, co-produced, can manage demand and are financially sustainable.

The Advanced Practitioner advisor role is responsible for the day to day leadership and management of the Council’s CHC (Continuing Healthcare) Hub, ensuring the delivery of the responsibilities of the Local Authority within the Continuing Health Care National Framework.

The post-holder will support the designated staff with implementation of the Care Act, the NHS Continuing Healthcare National Framework and other relevant legislation including changes associated with the social care reforms and CQC quality assurance framework. They will be leading developments in quality of practice, ensuring that practice is compliant with legislation and delivers the Local Authorities responsibilities. As part of this they will support the development of learning and development opportunities which should be aligned with professional development requirements.

The post holder must be prepared to represent Dorset Council at both operational and strategic levels across NHS England and Integrated Care Boards in developing policy and improving collaborative working with partners across the local (and national) health and social care system. The role will include responsibility for attendance at Independent Review Panels.

The post-holder will support the delivery of the practice and service changes required to make Dorset Adult Social Care a modern and progressive service which meets the outcomes of people in Dorset. They will role model through their leadership the behaviours’ that are required for this transformation. The role will lead change to embed co production in practice and valuing the diversity and experience of the adult and their family to improve their outcomes.

It is expected that the post-holder will support the strategic development and implementation of specific workstreams within the transformation programme. They will contribute to the design and implementation of new operating models for adult social care. They will also contribute to the leadership of the Directorate work, working closely with colleagues in other workstreams to ensure a collaborative approach.

The post holder will be involved in audit work, quality assurance and budget meetings and will provide regular performance and quality reports as required.

The post-holder will be expected to participate in Directorate meetings and contribute to whole council working groups. They will be required to liaise effectively with local elected members and have an ability to work collaboratively with partner organisations within Dorset ICB and a wide range of stakeholders.

**Further Information**:
This position has a significant travel requirement. This means that there is a requirement for a vehicle (or transport deemed to be suitable by the county council) to be available on most working days in order to carry out normal duties.
