The Fylde and Wyre Intermediate Care South team is based at Westview Health Village, Broadway, Fleetwood.
The service has 2 parts - Reablement with Therapy within patients’ homes and Thornton House Rehabilitation Unit which is an 18 bedded Unit.
The team consists of experienced and dynamic Occupational Therapists, Physiotherapists, Physiotherapy Apprentice, Therapy instructor and a Team Administrator.
The team is fully integrated with Social Care and care providers to assess, provide treatment and equipment for patients during patient therapy episodes referred to by Social Services and the hospital discharge teams.
The team awork alongside Social Care Colleagues, District Nurses, Reablement North Occupational Therapists, Compass Medical (FCMS), Thornton House Carers and an external care agency-Cherish.
Our aim is to encourage people to regain independence and confidence to take part in daily activities with a person-centred approach. As an experienced Band 4 Therapy Instructor you will work within the community delivering therapy support to individuals requiring rehabilitation and assessment of their mobility in their own homes and within an 18 bedded Rehabilitation Unit.
You will need to be enthusiastic, forward thinking and have a problem solving approach to patient management; with excellent communication/ interpersonal skills and demonstration of the ability to support other team members. Experience of working in the NHS and at this level would be advantageous.
This post will provide support to both Occupational Therapy and Physiotherapy, and will need specific skills in both areas.
- Responsible for assessment, intervention and discharge planning of patients assigned following triage by both Occupational Therapy and Physiotherapy.
- Gain informed consent from the patient prior to assessment, fully discussing proposed treatment, encouraging questions and giving them the choice as to whether to proceed or not. To inform supervisor of any concerns about capacity.
- Use technical and creative skills to assist in the provision of therapeutic activities for individual and groups of patients.
- Responsible for managing delegated caseload on a day to day basis including prioritisation of tasks.
- Work in the community as a lone worker in line with Trust and departmental Lone Working Policy.
An exciting opportunity has arisen within the Intermediate care team for a permanent part time - 20 Hours Therapy Instructor.
Successful applicants will have access to clinical supervision and in-service-training.
The hours are to be worked over 5 days Monday to Friday between the hours of 8.30-4.30pm. This can be condensed over 3 days and to meet the needs of the service. This post is part time and the working hours will be discussed at interview.
The post holder will work within a multi disciplinary team under the long arm supervision of the occupational therapists and physiotherapists. You will be responsible for managing and prioritising a delegated caseload to assess and provide holistic care working along side individuals to support them to meet their identified goals.
TRAINING, KNOWLEDGE AND EXPERIENCE
- Knowledge and skills acquired through previous experience working in Occupational
Therapy and Physiotherapy and extended through Continuing Professional
Development.
- Undertake competency training for specified duties, both within Occupational
Therapy and Physiotherapy.
- Attend mandatory training including Moving and Handling, Basic Life Support,
Consent, Fire, Equality & Diversity, Child Protection Awareness etc.
- Demonstrate an awareness of policy and procedure affecting the post.
CLINICAL RESPONSIBILITIES
- Undertake assessment and intervention within designated caseload. Working with
people on an individual and group basis.
- Provide general advice to patients, relatives and carers to enable understanding of
the aims of the prescribed intervention, ensuring there is a consistent approach to
patient care and offering information on community services.
- Negotiate and agree functional treatment goals with the patient using a range of
technical and creative skills.
- Formulate treatment plans, including exercises, assistance with gait, moving,
handling and positioning of patients, to enable them to participate in treatment.
Enable patients to overcome barriers and improve ability to perform activities of
everyday living. This may include confidence building.
- Demonstrate exercise programmes to carers in residential setting and patients own home and encourage participation as indicated.
- Evaluate patient progress, reassess and alter treatment programmes if required in discussion with your Supervisor where necessary.
- Be responsible for the agreed discharge of patients following completion of an episode of care.
- Use problem solving skills to adapt intervention and resolve difficulties that arise.
