An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join our Surgical Assessment Unit team at Colchester Hospital.

SAU is a fast paced environment offering same day assessment, investigation and diagnosis for patients within the surgical specialities. Referrals are taken directly from GP’s, A&E and other specialities

You will be required to have the relevant Qualifications and hospital setting experience to be eligible for this post.

You will need to show a willingness to undertake extended roles.
- Ensure that safe, effective and holistic patient care is delivered
- Assist patients to fulfil the activities of daily living
- With direction of the registered professional, assess, plan, implement and evaluate individual programmes of care in partnership with patients and relatives/carers
- Accurately observe patient’s condition and initiate appropriate action as required escalate to the registered professional
- If appropriately qualified administer medication safely to patients
- Actively participate in effective discharge planning
- To be suitable for this post you will need to have excellent communication and interpersonal skills with the ability to work on your own, but also as an integral part of the SAU team

- To meet the needs of the Trust with regard to Induction, mandatory and statutory training
- To meet the direct and indirect care needs of patients under the delegation and supervision of a Registered Nurse
- To ensure high standards of patient care at all times, in line with Trust policies and guidelines
- To work as an effective member of the nursing and wider multi-professional healthcare team
- To work within the boundaries of the role of the HCA supporting more junior colleagues as required and refer to the Registered Nurse as appropriate
