We are looking to recruit are looking to recruit a professional, conscientious, individual to become part of our Adult Care Services Team as Progress Chaser for our Community Hospital based in Midway Unit - Langley House, Watford, Hertfordshire.
Previous experience is desirable, however full training will be given.

**The duties and responsibilities of the role include**:
- Supporting the patient’s pathway thereby reducing any delays for the patient and working toward the patients Expected Discharge Dat
- Chase up and progress actions to deliver the timely discharge of patients to meet their expected date of discharge

Other duties include;
- Meeting patients care needs under the delegation and supervision of a Registered Nurse or Therapis
- Ensure the delivery of high standards of patient care at all times, in line with Trust policies and guideline
- Work as an effective member of the multi-disciplinary team and refer all other matters outside of their scope of practice to the Nurse in-charge, registered nurse or Therapy team
- Observe high levels of confidentiality at all time
- Manage and prioritise own workload within deadline
- Record all types of information accurately and have the ability to produce and maintain documents and spread sheets
