**About Us**

The Society of St James (SSJ) has been working in Hampshire for over 40 years, delivering high impact, person-centred, accommodation services to adults and young people experiencing homelessness, problematic substance use and mental health problems. **The Role**

This service forms part of a new interagency service that has been commissioned to deliver a variety of intensive interventions to homeless people who need support to access substance misuse treatment and interventions, in order to support their move through the Rough Sleepers Accommodation Pathway.

The team will deliver a variety of intensive in-reach interventions at key points across the city including the interim homeless accommodation, Rough Sleepers Hub and housing pathway accommodation locations (including Private Rented Sector and Houses of Multiple Occupancy). The team will work with the Rough Sleeper Outreach Team, the Homeless Health Team and the Portsmouth Recovery Services to provide wrap-around support and treatment interventions.

The team will be operating 7 days per week, including evenings and bank holidays.

The role will support the team Manager to offer line management and supervision to a team of Complex Needs Workers, and will also provide case management and offer psychologically informed interventions to a small number of service users.

We are looking for a person with experience of delivering and supervising case management, psychosocial interventions and practical support. The interventions will be aimed at supporting people to increase awareness of their behaviour and its impact on their quality of life, increase their motivation to make behaviour changes, and support them to access specialist medical and psychological interventions where appropriate.
- Support the Manager to lead a multi-disciplinary team to provide person-centred interventions to service users with a range of needs.
- Provide line management and daily supervision, including probationary reviews, formal and informal supervisions, appraisals and coaching to the team.
- Work intensively and consistently with a small caseload of people, supporting them to identify and overcome barriers to change and improve their quality of life.
- Carry out strength-based assessments and co-develop individual recovery plans to assist move on.
- Work alongside partners in the Inclusion team to case manage and support clients on substitute prescriptions.
- Work closely with the PCC Housing Needs and Advice team to ensure that current accommodation provisions are optimal for the clients.
- Arrange and co-ordinate clients’ access to detox and rehab where appropriate.
- Make referrals where appropriate and/or signpost service users to other services in Portsmouth.
- Be responsible for the ongoing monitoring of individual recovery plans for people on your case load, including liaising with colleagues across the service to ensure that service users receive continuity of support.
- Work in partnership with colleagues and other agencies, both statutory and voluntary, to provide effective support and achieve the best outcomes for service users.
