Assist in providing specific diagnostic and therapeutic care to patients, in support of and supervised by Registered Nurses.

The overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement.

To contribute to delivering a philosophy of care which is patient focused, promotes high standards of quality and cleanliness facilitating an environment which is conducive to best practice in all aspects of patient care and rehabilitation.

Working in support of the multi-disciplinary team in the acute stroke setting.

Provide care to patients to meet their individual needs with the focus on the promotion of independence.

To enhance and facilitate the stroke patients’ clinical pathway including the maintenance of therapy provision following agreed carer plans.

Working on the TIA clinic on the ward to provide continued patient care.

Participate fully as a team member, sharing knowledge and supporting colleagues to promote a cohesive department team and the achievement of team objectives.

Will support Student Nurses and new Healthcare Assistants to achieve set objectives.

Keep clinical areas clean and tidy, to maintain a safe and pleasant environment and contribute to the control of infection in clinical work areas. Assist in the monitoring of and restocking of supplies and promote effective and efficient use of resources.

Carry out assigned record keeping as directed, with Registered Nurse counter signatory and support qualified staff to promote nutrition.

Report untoward incidents, accidents and any other relevant information to the Ward Leader or Registered Nurse.

Whilst under supervision will be required to record information both clearly and accurately (verbal, written or computer based). Report to a Registered Nurse any abnormal findings Participate in a network of highly complex information including that of a sensitive nature.

Maintain confidentiality of all patient information.

Adhere to the ward procedures for the correct use of equipment and assist in maintenance of equipment as directed.

Be aware of and adhere to Trust policies and procedures as appropriate, as identified by the Ward Leader and qualified members of staff. Safeguard yourself and others working within Trust policies when handling dangerous and hazardous materials.

Monitor and replace used items of linen and laundry as necessary.

Monitor and maintain stationery documentation in appropriate areas.
Comment on policy and service development in order to re-design services in response to the demands of the National and Local agenda.

Demonstrate a courteous and helpful approach to patients, relatives and visitors to the Ward/Department supporting other team members to create an environment that is motivating, stimulating and supportive.
