As a **Wellbeing Support Practitioner**, you will play a crucial role in **implementing** the **7 Keys to Citizenship **framework within our organisation and supporting individuals in achieving their full potential. You will work closely with a diverse range of individuals, to **enhance** their **wellbeing** and **promote** active citizenship, including those with Learning disabilities, Autism and/or Mental ill health.

This position involves four one-hour daily calls, at approximately the following time slots: 8:00 AM to 9:00 AM, 12:30 PM to 1:30 PM, 4:30 PM to 5:30 PM, and 8:00 PM to 9:00 PM.

**Key Responsibilities**:

- Support individuals in building meaningful relationships and networks.

- Empower individuals to make informed choices about their lives.
- Assist individuals in identifying their goals and preferences.
- Provide information and support to help individuals make decisions that align with their aspirations.

**3. Contribution**:

- Encourage active participation and contribution to the community.
- Identify opportunities for individuals to share their skills, talents, and interests.
- Collaborate with community partners to create volunteering and employment opportunities.

**4. Rights**:

- Advocate for and uphold the rights of individuals.
- Educate individuals about their rights and responsibilities.
- Advocate for individuals to ensure they have access to the services and support they need.

**5. Purpose**
- Help individuals find a sense of purpose and direction in life.
- Work with individuals to set and achieve personal goals.
- Support individuals in pursuing their passions and interests.

**6. Recognition**:

- Acknowledge and celebrate the achievements and contributions of individuals.
- Organise events and activities that recognise the accomplishments of individuals.
- Highlight the positive impact individuals have on the community.

**7. Security**:

- Ensure individuals have a safe and supportive environment.
- Assess and address any safety concerns or risks.
- Collaborate with other professionals to provide a holistic support system.

**Skills and Experience**
- Level 2 or above in a Health and Social Care related topic and/or relevant experience
- The ability to work rolling 12 hr shifts (3 shifts week one and 4 shifts week two)
- Previous experience working with individuals with disabilities or in a similar support role.
- Strong understanding of the 7 Keys to Citizenship framework.
- Excellent interpersonal and communication skills.
- Empathetic, patient, and compassionate approach to supporting individuals.
