**Young Persons Practitioner**

**Salary**:£25,900 per annum

**Location**:Mold

**Contract/Hours**:Fixed Term Contract until 31 December 2024 (with possible extension), Full time - 37 hours per week

**A bit about the role**

Our unique service based in Mold offers support for individuals aged between 11 and 18 years old. We support young people at risk of criminal exploitation and serious violence by providing targeted intensive 1:1 support, family support, peer mentoring and group work to divert young people away from the harms caused by these issues.

To be successful in this role, you will need to have a keen eye for detail, be able to read between the lines and be sensitive to the needs of the young people we work with. As a Young Persons Practitioner, you'll work in a small but dynamic team, supporting the current practitioners and the young people they work with, working discreetly and building trust at their pace.

Due to the role requiring travel across Flintshire, a driving license and access to own insured vehicle is required.

**How you'll help to create brighter futures**
- Providing a first class service to young people based on an assessment of need
- Delivering specialist interventions with children, young people or families on a 1:1/2:1 basis, group or community setting.
- Supporting the team to transport young people when needed.
- Providing lead case/care planning management, including undertaking assessing, reviewing and managing risks
- Representing the service in community organisations, partner agencies and other external agencies.
- Retaining accurate records, using various IT systems to capture information and to produce written reports and present these as necessary.
- Acting as a young person advocate in your work.

The attached job description outlines all the requirements for our Young Persons Practitioner role, however some of our must haves are:

- Demonstrable experience working in a multi-agency social care or youth work setting
- A keen interest in well-being and mental health support strategies for young people
- Experience of working with young people with complex support needs, and ideally those within the youth justice system
- A positive and non-judgemental attitude to young people in conflict with the law
- Intermediate IT literacy with the ability to provide detailed reports through IT use
- Excellent communication skills, adapting your delivery to each individual
- Safeguarding knowledge and able to work confidentially with a sensitive approach
