EXPERT CLINICAL PRACTICE 1. Provide an evidence-based ophthalmic Nurse led service, which is responsive to users needs, to a cohort of patients referred to Ophthalmology. Care will reflect current local protocols and national guidelines and will be delivered through face-to-face consultation or via telephone communication on a day-to-day basis. 2.

Demonstrate advanced practice skills in the holistic assessment, planning, implementation and evaluation of care for the cohort of patients within the ophthalmic speciality. This will necessitate high levels of autonomy and advocacy, using analytical and judgemental skills in undertaking differential diagnosis to ensure the delivery of appropriate care. 3.Provision of overall ophthalmic advice to patients, parents, giving appropriate detailed instruction and making complex judgements regarding patients treatment and needs. 4.

Manage a cohort of patients and prioritise workload to meet patient and service need. 5. Undertake clinical competencies relevant to ophthalmology to demonstrate expertise in extended nursing roles. 6.

Facilitate and co-ordinate the process of informed choice for patients in the provision of treatment and care. Ensure that individual care plans are understood. 7. Act as a patient advocate to guide treatment and care decisions to ensure that patients individual needs are expressed and valued.

8. Advise on the complex disease and symptom management for patients within the ophthalmic speciality in both the inpatient and outpatient settings. Evaluate response to interventions and advise on the adaptation of management. 9.

Provision of comprehensive visual field assessment and analysis. Informing medical staff and patients of results, as appropriate, to aid in the diagnosis and treatment of patients under both ophthalmology and neurology directorates. 10. Administration of Ophthalmic drugs as per NMC guidelines.

11. Assessment of vision of patients with various degrees of visual impairment. 12. Bandage contact lens service for therapeutic rehabilitation of a range of ocular abnormalities.

Facilitate use of bandage contact lenses for patients with complex and painful corneal conditions. 13. Colour vision assessments 14. Act as a resource for health care professionals working within the Trust and primary care, providing specialist professional advice and support.

To revolutionise telephone triage and develop junior staff within the role. 15. Work autonomously in the development and delivery of nurse led or parallel clinics in line with National guidance and local service need. 16.

Nurses with IP can prescribe within their scope of practice. 17. Post cataract surgical reviews, the identification of significant post-operative complications, the counselling of patients on further management (including YAG laser) and to recommend further surgery (second eye), discharge etc. 18.

To be able to inject or working towards administering intravitreal injections. 19. Macular assessment clinics for patients receiving anti-Vegf treatment. The post holder will interpret OCT imaging and perform Volk retinal assessment to inform supervising medical clinicians of treatment options.

The post holder will be conversant with anti-Vegf treatment and assessment protocols. Specialist Nurses with experience, will also work alongside the consultant in assessing the patients who are new wet AMD referrals. 20. To teach and supervise nursing and medical staff where necessary.

Teaching may be on-going during clinical sessions or involve planning and providing training for larger groups of staff, with formal presentations. Teaching to other members of ophthalmology team would not be likely to be more than 3 people at any one time. 21. To make a significant contribution to the departments research activity and to participate in the ophthalmic aspects of various multi-disciplined research studies.

The post holder will be required to collate information and data for department research and audit and would be expected to be involved in supporting/discussing new initiatives and ideas in research. 22. To be actively involved in the departments agenda for effective clinical governance by ensuring professional standards for clinical practice are maintained, and regular audits are initiated and undertaken. 23.

To undertake a range of administrative duties, necessary for the smooth running of the optometry service and as required by the Lead nurse. 24. To adhere to all Trust (and departmental) policies and procedures and to report health and safety matters, clinical incidents and act responsibly in identifying and assessing risks. 25.

To be responsible for ensuring mandatory courses are attended.
