**Responsibilities**:

- Observe and report any changes in the patient's vital signs.
- Maintain accurate records of pulse and blood pressure levels, etc.
- Rise at a designated time each day to check patients.
- Report any signs of patient deterioration to the doctor.
- Bend, stoop, or reach in order to complete patient care tasks.
- Be confident in writing person centered care plans.
- Be competent with the administration of medication.
- Have good working relationships with professionals visiting the home.
- Help to resolve any complaints raised by family, residents or visiting professionals.
