Transitions Project Lead

You will contribute to the development of the AADS pathway, by undertaking a review of the current transitions pathway in place for young people with suspected autism. Working with people and family carers with lived experience, identifying what works well, the outcomes being achieved, and gaps in the current offer. You will make recommendations and support the implementation of a pathway for young people transitioning from children to adult services with suspected autism who require a diagnostic assessment. The work will complement the overall development of the Autism pathway.
The Adult Autism Diagnostic Service is currently engaged in exciting service development initiatives, working with colleagues across systems to improve the experience of service users. The team are dedicated and passionate about people with autism and ensuring that services are evidenced based and of the highest quality. The AADS are a friendly and welcoming team who sit within the wider Learning Disability services.
1. To work in direct partnership with people with lived experience and their family carers.
2. To work with children’s services and adult services to explore pathways for autism diagnostic services.
3. To undertake engagement, gathering the views of stakeholders including their understanding and experiences of young people transitioning from children to adult services with suspected autism. To include but not limited to CAMHs, CAMHs NDT, AADS, Autism Beds, CMHT and GPs.
4. To undertake a “live” review of a young person of transitions age with suspected autism.
5. To analyse data and information provided as part of the networking, reviewing and engagement work that provides quality and performance information and identifies key themes.
6. To understand and articulate the current approaches established within the local community.
7. To review the systems understanding of supporting young people with suspected autism.
8. To review how information is shared across the system to ensure that all known risks are acknowledged to inform appropriate and proportionate intervention from the support system.
9. To make recommendations and support implementation of pathway between children and adult services for young people with suspected autism who are waiting for a diagnostic assessment.
10. To map out services and pathways available for young people with autism.
