A new and exciting excellent career opportunity has arisen for a highly motivated qualified Senior Advanced Care Practitioner (ACP) to work within our Integrated Primary Care Division which includes daytime and Extended Hours (Enhanced Access) primary care.

You will deliver telephone and to face to face care alongside our multi professional team for a broad range of conditions for patients registered at PC24 practices but also those being referred into the Enhanced Access service from external sources.

Working with the senior Nursing & Quality team, the post holder will support and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will lead the implementation of relevant clinical guidelines and contribute to clinical policies and procedures.

**Overview**:
They will lead team of ACPs across PC24 Integrated Primary Care and to work autonomously as part of the ACP team using the expert knowledge base, complex decision-making skills and clinical competencies of advanced practice.

They will be responsible for line management of the ACP team which includes delivering yearly appraisal with support from the Organisations Professional and Medical ACP leads when needed.

They will work closely with the Quality Governance and Senior Nursing Teams to investigate and assist in resolving complaints and incidents related to advanced clinical practice. In addition, they will act as a professional role model for all members of PC24

**Hours**:37.5 hours per week, Monday to Friday 9am to 5pm

**About You**:

- Minimum of 5 years post registration experience in profession
- MSc in Advanced Clinical Practice
- Non-Medical Prescriber - V300
- Clinical Examination and Diagnostics Level 7
- Experience of working in urgent care / primary care or similar setting
- Long term conditions qualification
- Mentor/teaching qualification
- Trained clinical supervisor
- Regular Prescribing Activity
