Social Prescriber

Have you got experience of helping others to help themselves and a passion for supporting people?

We are looking to recruit two Social Prescribers for the Trowbridge Primary Care Network based at Trowbridge Health Centre. You will be a key member of the multi-disciplinary team, providing support to patients to empower them to take control of their health and well-being. The support will focus on what matters to the patient and taking a holistic approach to their individual health and well-being, connecting patients to community groups and statutory services for practical and emotional support. You will support goal setting, personal growth, and behaviour modification, to build resilience and social connectedness, reduce inequalities and help patients to have greater control over their health and lives.

If you have good local knowledge of local community activities and the initiative to find out more about the range of non-medical options available, then being a Social Prescriber could be the role for you.

You will need to be well organised, able to manage a caseload, have good IT skills and be able to keep accurate records.
