An ideal opportunity has arisen to be part of a well-established Community Matron and Community Nursing team in Reading
- The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive self-management of long-term conditions with the aim of avoiding crisis resulting in unplanned hospital admission
- The post holder will work closely with the Community Nursing Teams to manage complex patients as well as the wider multi-disciplinary team in a variety of settings to improve health outcomes and quality of life for patients with multiple chronic diseases.
