**Your new company**
Hays recruitment are proud to be working in partnership with a public funded health care provider as they require motivated and reliable individuals to offer in home support to vulnerable adults in Warrington.
**Your new role**
AS an outreach support worker you will offer in home support to vulnerable adults that have been recently discharged from hospital, ensure that each individual in your care feels safe, cared for, and valued in their local community whilst they transition toa private domiciliary health care provider. You will perform risk assessments and assist in the development of care plans so to ensure a smooth and successful transition. Your duties will include but are not limited to personal care, administering medication,meal preparation and feeding, domestic household duties, and taking a key interest in the interests of the service user.
**What you'll need to succeed**
- Have a valid UK driving license
- Be an empathetic and caring individual
- Be willing to learn
