Our award-winning #TeamSharedLives are expanding and looking for a **Family Placement Assessor** to join their team in the central belt on a** full-time, permanent basis**. It would be great if you have experience of completing assessments but this isn't essential as we are more interested in you as a person and what you can bring to our team.
This role will be based out of our Airdrie office as part of our hybrid model of home and office-based working for your first 6 months.
**The Role**
Cornerstone’s **Shared Lives and Short Breaks** service provides both transitional and long-term placements to people aged from 16 years old who require care and support due to a disability, health condition or support need. Shared Lives Carers and their families provide support and accommodation.
As the **Family Placement Assessor**, you will be responsible for managing Shared Lives placements. This will include the recruitment and assessment of new Shared Lives Carers, including the preparation and presentation of the required documentation to the approval panel. Once the Shared Lives Carers have been approved, you will also then facilitate the matching process for new placements, including the preparation of Personal Plans, and providing ongoing support and development for Carers within their role. You’ll continuously monitor placements, managing and reporting issues, and ensuring an exceptional standard of care is being provided, which meets the needs of the supported person.
**About You**
As extensive travel will be required across the central belt and will include monthly meetings in Ayrshire and every other month in Dundee it is **essential** that you hold **a full, UK driving licence **and have access to your own vehicle.

- be empathetic with a warm, friendly personality and a positive, can-do attitude
- be able to demonstrate exceptional communication skills and patience
- be able to think on your feet and use your own initiative
- have demonstrable experience in comprehensive assessment and reporting
- be able to build and manage good professional relationships
- have knowledge of the relevant legislative framework of ‘Adult Placement Services’
