**Key Accountabilities**: 1. As part of a Lymphoedema service, providing and delivering a high standard of lymphoedema care to patients with cancer and non-cancer lymphoedema, ensuring effective communication in partnership with the patient and their carer. At all times maintain the privacy, dignity, comfort and safety of the patient. 2.

To reassess and evaluate established programmes of lymphoedema care to patients, or make adjustments; monitor patient experience. 3. Assess the wider needs of the patient and their family providing advice and information on symptom control and psychological support issues, referring and signposting patients and family members to other services/multi-disciplinary teams both internally and externally as required. 4.

Work collaboratively and in partnership with other health professionals, supporting clinical staff within the organisation to provide lymphoedema care, acting as a link between IPU and the community services utilising high levels of communication between all departments. 5. Participate in audits, research and quality of service monitoring as directed by the Lymphoedema Specialists Nurses. 6.

Act as advocate for individual patients and the client group. 7. Provide appropriate education to patients and their families; provide them with advice and emotional support in the Hospice or at home, as appropriate. 8.

Maintain clear and accurate documentation. 9. If appropriate, cover agreed elements of the lymphoedema administrator role in their absence, including meeting and greeting patients, answering the telephone and making clinic appointments when needed. 10.

Keep up-to-date of developments within lymphoedema, attending appropriate lymphoedema and palliative care courses as available. To keep an activity record, in a personal portfolio, of professional contributions and time spent working on activities related to lymphoedema.
