Imagine now needs a Peer Support and Wellbeing Officer to deliver a valuable service which will improve the wellbeing of people experiencing poor mental health. Using health coaching skills to support people to develop knowledge, skills and confidence to reach their own health and wellbeing goals.

The main duties of the role are to:

- **Improving Physical Health and Wellbeing**_
- To deliver health and wellbeing programmes using identified course materials, following a recommended programme plan
- To use skills and experience to encourage people supported to contemplate improving their physical health
- To assist people supported to establish Physical Activity Plans identifying their own health and wellbeing goals and review progress against goals
- To assist people supported to use tools to implement supported self-management
- To use 5 Ways to Wellbeing Toolkit and PHE resources to raise awareness of the impact of poor physical health on mental health and assist people supported to improve Wellbeing
- To use knowledge and skills to assist people supported to make healthy changes including losing weight, getting active; stopping smoking; reducing reliance on substances and drinking within recommended limits
- To identify issues impacting on mental health and signpost to services
- To encourage family members to take part in programmes including sharing resources; establishing physical health plans; attending sessions and events
- To develop trusting relationships and deliver person-centred flexible support
- To assist people supported to prepare and register for annual health checks and follow guidance
- To encourage participation in sport and leisure, volunteering pursuits which involve physical activity and to introduce people supported to community gardening projects
- To increase digital skills and connectivity
- To ensure all administrative, recording and reporting tasks are undertaken in an appropriate and timely manner, ensuring confidentiality at all times in line with imagines Confidentiality policy
- To follow referral, assessment and risk management processes and to actively promote and market the service.
- **Facilitating Peer Led Groups**_
- To establish and facilitate a range of self-managed Peer Led Groups including face to face group and online sessions in accordance with service user’s preferences.
- To facilitate Wellbeing Workshops and support Peer Mentors to develop skills and confidence to facilitate Peer Led Groups and provide ongoing support and direction.
- To encourage people supported to access socially inclusive opportunities independently from group activity

**Person Specification**:

- NVQ Health and Social Care, Level 3 (or a willingness to work towards)
- RSPH Level 2 Understanding Health Improvement or equivalent (or a willingness to work towards)
- RSPH Level 2 Award in Nutrition for Health (or willingness to work towards)
- Motivational Interviewing Practice and Process Course or equivalent (or a willingness to work towards
- Making Every Contact Count and 5 Ways to Wellbeing Training (or a willingness to work towards)
- Very Brief Advice on Smoking or equivalent (or willingness to work towards)
- Exercise and Mental Health Open University online course (or equivalent or willingness to undertake this
