Discharge Facilitator

An exciting opportunity has arisen for a Band 6 Discharge Facilitator to work in the Rehabilitation Unit for the Elderly at Wrexham Maelor Hospital.

The Discharge Facilitator post holder will work as part of the multi-disciplinary team in order to plan, organise and facilitate the patient’s journey ensuring that a safe and timely discharge plan is facilitated. The post holder will work closely with all members of the MDT team and will be supervisor to the Progress Chaser within the department.

Applicants for this vacancy need to have experience of the discharge planning process and have a understanding of the procedures in place to support patient flow and facilitate the safe and successful discharge of patients to the appropriate setting.
