**About this opportunity**
- We have an exciting opportunity for an enthusiastic and motivated individual to join the Learning Disability, Autism and Transition Social Work Team in South Lakes and Eden, Westmorland. We are a creative and supportive team working with young people and adults who have a learning disability and/or autism.

- As an entry level Practitioner, you will be assessing the needs of extraordinary individuals and their carers to ensure the development and delivery of appropriate person-centred, self-directed support. Using relevant legislative processes to qualify your practice.
