As a Health Adviser for SEND (HAS) you will liaise with children, young people, and their families, who aren’t currently accessing community health services, to undertake a ‘light touch’ (virtual) holistic assessment of their needs. This role will include review of electronic and paper health records, and communicating with system partners across education, health, social care, for example, GPs and school SENCO. It may also involve making onward referrals to health services for a full assessment.

You will work closely with the ICB Designated Clinical Officers to develop and implement this new service, whilst ensuring there are robust governance, reporting and quality assurance mechanisms in place to ensure ICB compliance.

BSW ICB is a statutory body which brings together NHS organisations with local authorities and other partners to work to improve population health and establish shared strategic priorities. The ICB oversees how money is spent and makes sure that health services work well and are of high quality. It brings together hospitals, primary care, local councils, hospices, voluntary community and social enterprise (VCSE) organisations and Healthwatch partners in our local places, Bath and North East Somerset, Swindon and Wiltshire.

BSW ICB is part of BSW Together Integrated Care System(ICS),which is made up of NHS and local authority organisations working together. The five core pillars of BSW Together are: Leadership, Learning, Inclusion, Innovation, and Improvement. As an ICS, BSW Together has four key purposes to: improve outcomes in population health and healthcare; tackle inequalities in outcomes; experience and access enhance productivity and value for money; and support broader social and economic development.

The post holder will lead a newly formed team of health professionals who are responsible for providing health advice and information for children and young people aged 0-25 with Special Educational Needs and / or Disability (SEND) as part of their Education, Health and Care (EHC) needs assessment or as contribution towards their EHC Plan Annual Review when the child or young person isn’t already known to community health services.

The role, and the duties outlined below will ensure the ICB is compliant with its statutory duties in accordance with the Children and Families Act (2014) and SEND Code of Practice (2015).

The post holder will work collaboratively with the child, young person and their family to gain their views and opinions, review electronic medical records and liaise with other Professionals who know the child well such as the school SENCO or GP to provide a ‘light touch’ holistic assessment within the statutory timeframes which can be used to inform the EHCNA and, where necessary, to populate Sections C and G of the EHCP.

The Health Adviser for SEND (HAS) will support and contribute to the Quality Assurance work being led by the Designated Clinical Officers (DCOs) to ensure that the health elements of EHCPs are accurate, concise and up to date. This will also include, but not be limited to development and implementation of health report templates and agreed pathways for information sharing.
