Life Skills Hub has an amazing opportunity for Specialist Wellbeing Practitioners to join our dynamic multi-disciplinary team You will have the opportunity to work as part of a great team and be a part of working with and for some amazing young people and families, working across Hull & East Riding and 0-25 years old.

You will be given the opportunity to develop your skills within our multi-disciplinary team including relevant training and working with some amazing colleagues from all different walks of life.

**About Us**

Life Skills Hub offers specialist wellbeing support across a range of internal and external settings and within the community with a view to improving positive wellbeing for children, young people and adults aged between 0-25 years old. We have a flexible and relational approach and a team of multi-disciplined staff to support our students & their families. We offer 1-1 and small group sessions and work across a range of organisations such as; schools, nurseries, colleagues, social care and local authorities across Hull, East Riding and the wider area. We support students to develop their wellbeing, social skills, confidence, life skills and support them with educational readiness and transition onto a future setting, as required. In September 2024, we hope to open a specialist SEMH independent school for young people in years 6-11 inclusive

We are recruiting for multiple Specialist Wellbeing Practitioners to work flexibly across our services. Our current availability is for our Intense Outreach Service - this involves working with children and young people at their home, school, community who have significant behavioural needs stemming from SEMH needs including high anxiety, trauma, and SEND. You must have excellent behavioural management skills, and be able to de-escalate situations using a calm and collected manner. You must be able to be assertive, when needed, and be able to stay calm when the young person may be in crisis or deregulated. You will be supported with this approach, through CPD training, supervision and we match students with practitioner's skills and needs. You must be a resilient person, who is open to change and flexibility to work across a wide range of ages and needs. You will be confident and believe in your own abilities. Experience of working with complex SEND needs, SEMH settings, or managing high level behaviours is desired.

**The Role**
- Working flexibly across one or multiple specialist wellbeing services within Life Skills Hub across Hull and East Riding working with students 0-25 years old, commonly with complex SEND needs and/or lived experiences and/or high anxieties
- Working on a 1-1 and small group basis to engage, interact and motivate children, young people and adults to promote their; independence skills, life skills, confidence, social skills, engagement in the community, readiness for learning, transition onwards to their next provision, when required
- Forming an excellent rapport with students, families and professionals, attending multi agency meetings as required
- Completing paperwork such as intervention logs, planning sheets, AQA evidence trackers and information on the student's individual file
- Be competent and comfortable being the lead practitioner for the student and undertaking planning and an understanding of the needs and next steps for the young person
- Driving to/from appointments, including home visits, within the community, working in schools, LSH sites
- Contributing ideas, passion and insights to the multi disciplinary team
- Supporting students in high levels of distress and emotional de-regulation
- Support students who may exhibit higher levels of risk, safeguarding and welfare concerns
- Attending supervisions, team meetings, CPD events, training and multi agency meetings regularly
- Using psycho-education techniques to help support the student with their anxiety/copying strategies
- Using and developing a range of person centred resources
- Navigating and inputting onto a range of documents including word and excel, and our computer software
- Managing complex behaviours, looking for the antecedent and using a trauma informed approach
- Using skilled de-escalation strategies, and managing complex emotional needs, whilst staying calm

**The Person**
- An all round team player, contributing to the needs of the students, families, professionals and colleagues
- Confident in their abilities to manage, support and progress next steps for students with a wide range of complex SEND needs and ages. Students will often present with high levels of anxieties, and sometimes demonstrate risk taking behaviours so it is essential the practitioner can remain calm in heightened situations and manage situations effectively. Practitioners need to be resilient
- Feeling comfortable with asserting boundaries whilst providing a nurturing, person centred and relational approach
