**“Contract starts from 02/10/2023 until 29/03/2024”.**

**Job Description: -**
- Provide support and assistance to individuals and families in need.
- Supporting families with children between 0 and 18.
- Conduct assessments, Completing assessments/identifying support needs/sourcing support from partner agencies and services/monitoring progress of support through regular multi-agency meetings..
- Collaborate with a team of professionals to ensure the well-being of clients.
- Advocate for the rights and needs of vulnerable populations.
- Assist with the coordination of services and resources.
- Promote and represent positively the work of the FACT Service with all partner agencies.
- Must have HCA level 3 qualifications (A levels, HNCB / NVQ 3)
- Knowledge of safeguarding and promoting welfare of children and young people.
- Previous experience in similar is required.
- Own transport is required.

- Temporary contract
