Senior Mental Health Practioner

assist in the overall delivery of care in a care coordination role to patients within the mental health team at HMP Risley and HMP Thorncross You will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for mental health. This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
The successful applicant is required to demonstrate a “can-do” attitude, be resilient and possess excellent organisational skill. An ability to work well as part of a multidisciplinary team is essential.
Healthcare services operate 7 days a week, and the successful applicants would need to be flexible in their approach to meeting the demands of the service.
The days within the prison service are varied and there is a wide range of training and development opportunities available.
By being involved in multi-disciplinary and inter-agency liaison you will support Effective Care Co-ordination and the formulation of risk management strategies for those identified with complex needs. As well as being involved in the multi-disciplinary and inter-agency liaison to support and/or facilitate assessments as part of the secure services care-pathway.
It is also important as a Band 6 Nurse to make sure you are able to respect the rights of patients whilst upholding their privacy and dignity whilst also making sure any issues that will affect public protection either in or outside of Prison are raised to the relevant department
