By working at scale, we are better able to promote health and provide for the differing needs of the communities of local people and a balanced workload for our staff. It also enables us to employ a significant multi-disciplinary team including Pharmacists, Advanced Nurse Practitioners and Paramedics (for pre-assessment screening and home visits) all of whom reduce the clinical workload for GPs.

We have an exciting opportunity for a full-time 12-month fix term, social prescriber to join the organisation working across the Network. You would be working within our personalised care team, provide social prescribing and supported navigation for people with long term health conditions and their carers connecting them to services and activities, enabling active engagement within the community, the building of friendships and subsequent peer support and the development of new skills, all whilst supporting their mental wellbeing to support them to feel more confident about managing their health and wellbeing and the impact it has on their day-to-day life.

Essential skills:

- Experience in system development within the health service
- Knowledge of safeguarding all ages
- Local community knowledge would be ideal
- System 1 knowledge
