Provide a high standard of education service to outpatients with Type 2 Diabetes, through delivering education sessions using evidence-based practice. Work effectively as an individual and as part of a multidisciplinary team. Communicate accurately in writing and/or verbally with other health professionals. Ensure regular communication with the admin, clinical and management teams.
Prepare and deliver presentations to healthcare professionals using current evidence-based resources. Participate in peer assessments and observation sessions. Assist in the induction of new educating staff. Demonstrate a commitment to continuing professional development to improve patient centred care.
Participate in audits and quality monitoring. Proactively contribute to the achievement of clinical outcomes through individual and team effort. Conduct Risk Assessments as required for venues used in the delivery of the programme.
