An opportunity has arisen for a Band 6 Community Practitioner to join our award wining Older Persons Memory Assessment Service. This is a busy team covering the whole of Newcastle as well as the eastern borough of North Tyneside. Although the service is clinic based, a significant number of assessments are offered to patients living in a range of community settings across the locality.

We are looking for a suitably qualified person who is dynamic and motivated with a specific interest in the mental health care of the older people and a special interest in working with people with dementia. The appropriate knowledge and skills to communicate well with people who have cognitive problems and a good working knowledge of the signs and symptoms of dementia is essential.

You will work in a highly supported environment as part of the wider Older persons Pathway and will be provided with support to develop your knowledge and skills. Opportunities are available to develop as a non-medical prescriber, group work and in the delivery of diagnosis.

You must have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core Values.

However due to there being a clear pathway of care and excellent structure of what is expected in each of our appointments, this enabled me to learn and continue to learn and put my previous knowledge into practice into the MAMS service. I feel supported and valued within the team

Applicants for the post must have experience working with older adults, and knowledge of assessment and treatment processes appropriate to this client group.

The successful applicant will need to be confident, focused and well organized with the ability to be flexible and responsive as well as a demonstrating a positive attitude to teamwork.

To provide compassionate care that is based on empathy, kindness, respect, and dignity.

The successful applicant will work between 9am - 5pm, Monday to Friday
