At West Sussex we require experienced and motivational Senior Social Work Practitioner within Lifelong Services Transition Team.
Payrate: £38
This role includes:
- The Transition Teams are county-wide teams that offer a service to young people from the age of 16 who have lifelong disabilities. Lifelong disability transition services are for those young people with physical disability, learning disability and/or autism. The focus of the team is support people moving from childhood to adulthood and plan for the time after the person turns 18.
- Transition is a busy and exciting time for workers and young people. You will be expected to work within organisational, policy and legislative frameworks as you support young people to increase their independence and plan for the future.
