Assistant Practitioner

We are looking for a Band 4 Assistant Practitioner, on a permanent contract to join the team. You will be based in Honiton, but as a team we cover all of East Devon and there may be times when you will be required to work out of an alternative base within our area.

You will be expected to manage a small caseload of older adult clients, and provide evidenced based interventions for the patient, their carers and families and other professionals that may be involved.

We would also like you to develop good team links with the Care Homes across our area, supporting them to understand the needs of their patients and providing advise and guidance relating to this.

You will be available to offer additional support to Clinicians within the team, for patients who have more complex need and require additional interventions to maintain their well being.

You will be providing support for our patients on the Post Diagnostic Pathway, supporting clinician with their care, medication monitoring and signposting to other agencies as required.
