**Job overview**:
An exciting opportunity has arisen at Launceston & Stratton Community Hospitals for a Progress Co-Ordinator to join the ward teams. You will play an important role in supporting the clinical staff in facilitating safe and timely discharge from hospital for patients.

You will help provide a seamless service for patients which will include cross boundary and collaborative working with the multi-disciplinary team along with colleagues in Adult Social Care and other outside agencies in the community setting. Your role will be split during the week, working across both hospitals on different days

This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.

Launceston & Stratton Community Hospitals are busy community hospitals with a variety of services including a MIU departments, out-patient departments, Xray and in patient wards

This is a full-time post working 37.5 hours per week to include occasional weekends on a rota basis. Shift split over the week to be discussed at interview and agreed with successful applicant.

**Main duties of the job**:

- To assist with the efficient day to day running of a busy ward, providing administrative and clerical support to the nursing, therapy and medical staff providing a 7 day
- This post will be required to work some flexible hours to meet the requirements of the post, eg evening meetings with carers and participation in the Progress Co-ordinators weekend
- To meet patients and carers discussing consent, care and therapy plans and involving them in care planning for the ward stay and re-settlement.
- To ensure that patient’s needs are addressed, based on the care plan and co-ordinated in the most timely and effective
