The trust has been rated outstanding by CQC and as a trust we do everything we can to embody our values of ‘we care, we respect, we are inclusive’. ELFT’s vision to improve the quality of life for all those we serve has drawn us into new areas of work, including primary care. As we move forwards to provide more primary care services it is an exciting time to be part of truly transformational change across the whole health and care system. As part of the new primary care directorate within ELFT our team will be involved in leading triple aim population health quality improvement work. Quality of care is our top priority. Being the best requires continual improvement

Our values are: We Care, We Respect, We are Inclusive - so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations
The organisation and delivery of primary care services in ELFT is being re-visioned to reflect an aspiration to deliver services in a more integrated manner in collaboration with internal and external partners. To this end the portfolio of primary care services provided directly by the Trust is expanding and the model of service delivery is changing to include more direct participation of service users.
