Rehabilitation Assistant

**Overview of the post**:
1. To work as part of the multidisciplinary team within Woodlands Rehabilitation Neurological centre.
2. To deliver daily support and care for patients, including physical health care, personal care, transfer and mobility, eating and drinking, and leisure activity
3. To adhere to risk assessments and rehabilitation support plans written by nursing and therapy, promoting rehabilitation and person centred care
4. To maintain timely and accurate clinical notes and records
5. To contribute to maintain service excellence across all areas
**Key tasks and responsibilities**:
**Clinical**
1. To provide high standards of patient care and rehabilitation through awareness of individual needs and following of rehabilitation support plans
2. To facilitate the daily routine of patients within the unit and where indicated in the community of all aspects of their daily living rehabilitation goals
3. To assist patients with personal care and hygiene, promoting independence during these functional tasks
4. To assist patients with eating and drinking, in accordance with specialist Speech and Language therapy and Occupational Therapy guidelines This includes support of modified diets, and use of adaptive cutlery
5. To follow transfer and mobility plans, including the use of hoists and other transfer equipment used to support physical rehabilitation
6. To implement splinting protocols as directed by the qualified therapists
7. To be responsible for implementing orthotics rehabilitation support plans on a daily basis as directed by the qualified therapist
8. To carry out stretching plans, specific for individual patients under the supervision of the qualified therapists
9. To ensure optimum patient positioning, within chairs and bed, is maintained throughout 24 hour period
10. To be able to deliver exercise practice sessions and cognitive strategy practice sessions following guidance from qualified staff
11. To assist therapy staff with patients who require more than one person to give effective therapeutic treatments to
12. Undertake training to be able demonstrate, vital signs, PEG and continence care
13. To be aware of and follow risk assessment alongside rehabilitation support plans written by nursing and therapy disciplines
14. To support social activities for long term patients to access the community and leisure activities, to support emotional wellbeing goals
15. To provide person centred care, treating individuals with dignity and respect at all times
16. To maintain daily care records in accordance with legal and company standards, and ensure records are written in a timely manner
17. To upkeep the standards of the unit, including infection control, health and safety, and cleaning.
18. To support patients to upkeep the cleanliness and tidiness of their direct environment, and the wider centre
19. To provide daily social interaction with patients and family / relatives
20. To promptly report any concern regarding patients welfare or changes in presentation to the Nurse in Charge
21. To contribute to the maintenance of a safe and comfortable environment in accordance with health and safety standards
**Training responsibilities**:
1. Complete mandatory training as determined by the organisations policy
2. Be clinically competent in all areas of practice relevant to the post
3. Complete relevant competencies for all aspects of the role
**Governance and Professional issues**:
1. To take responsibility for own personal and professional development, through attendance at regular supervision
2. To maintain up to date mandatory training, including e-learning and face to face training
3. To comply with all company policies and procedures including Health and Safety, Safeguarding, Infection Control, and Fire Safety
4. To be aware of and work towards maintaining the national minimum standards set by the Healthcare Commission and CQC
5. To maintain professional standards, including adhering to confidentiality in all matters relating to patients and information obtained during the course of employment.
6. To ensure confidentiality is maintained at all times.

**Essential**
Basic numeracy and literacy
Willingness to undertake relevant training

Training in clinical moving and manual handling

**Experience**

Experience of customer care
Awareness of care within areas such as rehabilitation, physical disability, brain injury / neuro-disability, mental health, or other relevant area
**Desirable**
At least one year as a care worker
Experience in providing care for adults with brain injury and neurological conditions
Experience following clinical risk assessments and care plans
Experience working with a multidisciplinary team
