We are looking to recruit a suitably-qualified and experienced Speech & Language Therapist, to join our award-winning Community Stroke Rehabilitation Team across South Tyneside and Sunderland. You will work mainly in community settings on a domiciliary basis, to provide specialist advice, treatment and support for patients with disorders of communication and swallowing following hospital discharge after Stroke.

**Our multidisciplinary therapy, nursing and dietetic team works closely with inpatient and outpatient Stroke services, Outpatient Therapy and Dietetic departments, Social Services, and Primary Care. We have strong relations with local Stroke Association services allowing us to provide a seamless transition of care across the whole Stroke Pathway.
