**About the Role**:
This role is based in one location, but will include home and care-home visits. The working days are Monday to Friday and we would be happy to consider either full-time or part-time. The expected start date for this role is 03/07/2023.

The role will include the following duties:

- Care home ward rounds.
- Individual care planning.
- Acute response services.
- Running of clinics providing care for all presenting patients, from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care.
- Providing an holistic clinical service, by working collaboratively with the healthcare team and with support from GPs as required.
- Meeting the needs of the patients, implementing agreed management plans and following approved protocols as appropriate.
- Carrying out home visits and consulting with patients who present on the day, via booked appointments.
- Reviewing and acting appropriately on blood results, referrals and correspondence regarding patients.

If you are an experienced emergency care practitioner (ECP) / paramedic wanting a new challenge, we would love to hear from you. You will have the following skills and experience:

- Be a registered nurse or paramedic, with current HCPC or NMC registration and have post-registration experience, ideally with some being in primary care.
- Have completed ENP/ ECP minor injuries and minor illness course Level 6 and be a ALS/APLS provider.
- Be able to effectively assess, plan, treat and refer patients.
- Be able to evaluate patient care confidently.
- Have experience of working to protocols and guidelines.
- To exhibit excellent verbal and non-verbal communication skills.
- Have the ability to make clear decisions with confidence and communicate these effectively.
- Understand need to work within the parameters of one's own training and capability, according to the NMC Code of Conduct / Health and Care Professions Council.
- Have experience of autonomous practice working and multi-professional team working.
- Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children.
- The post holder is required to hold a full, clean UK driving license and have access to a suitable vehicle with a valid MOT and covered by business insurance.
- Be able to reliably commute to Harpenden, or plan to relocate before starting work.
