Our **Acute Crossroads Team** is responsible for the delivery of quality rapid response care to carers and their families, and who may need support to stay in their own homes to avoid hospital admittance or to support people to come home from hospital as soon as possible. The role is varied and often no two days are the same so a Support Worker role on our experienced Acute Care Team requires the flexibility, confidence and capability to deal with challenges and to step into situations quickly with new families.

**Carer Support Workers** **- Acute Team**

**West Kent** **(Tunbridge Wells, Tonbridge, or Sevenoaks areas)**

**Guaranteed Hours - 20 per week - weekdays**

**£13.80 per hour** / **£14,352 per annum for 20 hours a week**
- (FTE example is £26,551 per annum)_
