**Context**

Chronic pain is one of the most common long-term health conditions experienced by people living in Lambeth. Many long-term physical health conditions, including chronic pain, are best treated as a bio-psycho-social condition as they are closely related to a person’s mental and physical health, mood, social circumstances and behaviours.

**Job Purpose**

The Pain: Equality of care and support in the community (PEACS) project has three key work streams:

- Screening of patients, including through collection of patient reported outcome measures and 1:1 assessment
- Group-based psycho-educational intervention
- Support for patients to access social prescribing interventions within the local voluntary care sector

**Specific Duties**
- To successfully coordinate and support the delivery of the programme across primary care.
- Take the lead in managing the scheduling and coordination of the PEACS programme, including scheduling workshops, assessments, and follow-up reviews with patients, securing rooms, printing materials etc.
- Proactively call patients to encourage them to join the programme.
- Facilitate Workshops with residents in the community.
- Track patient progress, workshop and assessment bookings, collect feedback from patients, and clearly communicate this with relevant staff members.
- We work in partnership with a large number of statutory and community services and organisations across the borough. Provide ongoing support and follow-up for patients, linking them with appropriate services to meet their needs.
- Network to identify new services and community groups for clients to be referred to.
- To attend community events to promote the work of social prescribing and the PEACS service to develop strong referral links with local agencies and to encourage self-referrals.

**Essential Skills**
- Experience of supporting service users in a person-centred, holistic approach.
- Experience of data collection and using tools to measure impact of service.
- Experience of networking and proactively liaising with external agencies and community groups to maximise client support services and referrals.
- Knowledge of how the NHS works, including primary care.
- Knowledge of, and ability to work to, policies and procedures, including confidentiality, lone working, data protection, health and safety and safeguarding.
- Ability to actively listen, empathise with clients and provide non-judgemental support.

- Able to work in a flexible manner to meet the demands of a developing project.

**Salary**: £27,469.00 per year
