An exciting opportunity to work within the Hepatobiliary (HPB) CNS team to provides cancer care to patients at MRI hospital. The role will involve working alongside and supporting the CNS team in providing care and support to Hepato-pancreato-biliary (HPB) patients and their families across the whole cancer pathway from diagnostic tests through treatments to survivorship or supportive care. The postholder will have specific responsibilities in relation to triaging calls, and supporting the national personalized care programme, alongside working closely with the CNS team to ensure delivery of best care for this patient group.

The post holder will work as part of the GI Medicine and Surgery Single Managed Hospital Service within Manchester University NHS Foundation Trust Hepatobiliary (HPB) Specialist Team to support the CNS team in delivering specialist nursing care to Hepato-pancreato-biliary (HPB) cancer patients.

The Macmillan Cancer Care Support Co-ordinator role is an integral part of the Cancer workforce and involves direct contact with people living with and beyond cancer, to support them throughout their cancer care.

You will be a point of contact for patients and carers, supporting and co-ordinating their care. This will include assisting with personalised care, support planning (based on holistic needs assessment), treatment summaries, providing health and well-being information and support which includes personalised stratified follow up and helping to promote cancer awareness.

Providing support throughout the Hepato-pancreato-biliary cancer pathway including diagnostics/surgery/oncology, treatments/survivorship/best supportive care.

Communicating effectively with a range of individuals on various levels, eg: - patients, relatives, carers, other health / social care professionals and community workers.

Promoting equity for patients across the Greater Manchester footprint.

The post holder will be expected to work with all grades of medical & nursing staff and wider health / social care and community teams (including third sector and charity).
