As a member of the Neurology Multi-disciplinary team, the post holder will help facilitate the smooth running of the patient journey across all elements of the wider system by coordinating processes and monitoring the service provided to patients. The post holder will be responsible for the collection and monitoring of data pertaining to the service contributing to service development.
This post also requires the post holder to deliver accurate documentation and monitoring of high cost MS drugs and blood collection/result.
**Key working relationships**: Patients, Neurology MDT, Private providers, Nursing and Residential homes, General Practitioners, Practice nurses and Practice managers, Social Services, Community Health Services including Community Matrons and District Nurses, Other clinical and voluntary groups as required, Acute Hospital Providers and Carers and Carer organisations
Providing comprehensive administrative support to the Clinical Nurse Specialist team.
Ensuring that there is a point of liaison for service users receiving care coordination and interface with all professionals involved in service users’ care including primary, secondary and community services.
Coordinate the necessary assessments and appointments to facilitate the provision of timely care and treatment.
Booking routine and urgent clinic appointments, for patients in relapse, and arranging home visits
Support MS nurses by printing and delivering scripts to BDU and/or pharmacy
Plan and manage own workload in an environment of complex and competing activities/tasks
Contribute towards the implementation of service improvement and efficient processes across the department
Establish and ensure the continuation of effective communication between the MDT team, regional neurologists, regional nurses, GP’s and the patient to enable seamless provision of care.
To be responsible for inputting and collating data on the referral to the neurology Triage service.
Monitoring patients’ attendance and following up as appropriate.
Printing and sending patient letters and to any interested party.
Tasks and activities that support the neurology team and enable effective delivery of care to neurology patients commensurate with the position.
