Cambridge Care Company are looking for motivated and dedicated Care Planner to cover Newmarket, Haverhill and Bury St Edmunds.

You will be:

- Meeting with Service users to create Care plans tailored to their needs.
- Ensuring all Care plans are up to date and accurate.
- Based in our Newmarket office.
- Liaising with local authority and healthcare professionals in order to maintain Care Plans.
- Managing the ON CALL service.
