Care Navigator

**About the Role**:
We are seeking a part time care navigator to join our fabulous team at Comb Ford Surgery in Stowmarket on a part time basis with hours to be discussed at interview. Combs Ford Surgery is PMS Dispensing Practice with a List size of over 9,000 patients.

We are part of Suffolk Primary Care a Partnership of 8 practices that delivers care to 90,000 patients across Suffolk. SPC surgeries have a successful record of working collaboratively to deliver quality care to our patients. You would be working for an organisation focused on developing and nurturing staff and delivering excellent quality of care to our patients.

The main duties include:

- Provide comprehensive administrative support for the general practice and to act as focal point for patients and staff.
- Receive and assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way.
- Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors either in person or via the telephone.
- Facilitate effective communication between patients and members of the healthcare community.
- Handle patient complaints in accordance with practice protocols.
- To provide an efficient and accurate support to the secretarial team, clinicians & patients.
