**About the Role**:
The Grove Medical Centre is looking to welcome an experienced paramedic into our busy practice.

The main responsibilities of the role include:

- Provide assessment, diagnosis and treatment, at first point of contact, by attending to patients according to their needs either in the practice sites or in the patients' homes. Act as an autonomous clinician, with the support and guidance from the GPs.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
- Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
- Recognise, assess and refer patients presenting with mental health needs.

To apply, you must be registered with the HCPC, have at least 3 years experience post-qualification and be working toward level 7 capabilities. Funding requirements for the role stipulate that as part of the role you should work towards, and complete, the clinical pillar competencies of the Advanced Clinical Practice Framework.

Essential registrations, qualifications, skills and experience:

- Registered paramedic with HCPC
- Minimum of three years post registration with demonstrable experience of using diagnostic skills
- Experience of dealing with vulnerable patients
- Be working toward level 7 capabilities and to complete the clinical pillar competencies of the Advanced Clinical Practice Framework
- Understanding of health and social care processes
- Understanding of social determinants of health and how these can be addressed with patients
- High levels of health literacy
- Awareness of local services and resources for individuals, carers and families
- Must hold a current and valid driving licence
- Must have own vehicle and be willing to take out business insurance to cover usage (if not already in place)
- Able and willing to undertake visits to patients homes and care homes

Desirable qualifications, skills and experience:

- Advanced clinical practitioner or advanced level functional assessment qualification
- Demonstrate accountability for delivering professional expertise and direct service provision
- Assessor/mentorship course
- Evidence of working in a community/primary care setting
- Experience in teaching & supporting junior staff/students
- Knowledge of a range of technology and digital tools that can be used support health and wellbeing
- Sound understanding of disease prevention (primary, secondary and tertiary) and how patients can build prevention into their lifestyle
- Excellent knowledge of Microsoft Office and SystmOne
- Understanding of current issues facing the NHS
