**DUTIES AND RESPONSIBILITIES Clinical Practice**: Provide expert care for patients/clients and their care givers using the post holders expertise. Have specialist knowledge of patient group across a range of clinical issues, underpinned by the theory and practice acquired through study at masters level or equivalent experience. Act as a role model of expert nursing practice with specialist knowledge in diabetes. Establish, implement, and monitor trust guidelines, policies, and protocols with regard to the specific client group.

Initiate and accept referrals to and from other disciplines within agreed service specifications. Participate in multi-disciplinary reviews and assessments. Work unsupervised to advise, recommend and generate care pathways for client group, ensuring on-going evaluation and review. Be professionally accountable and support the team to manage their caseload.

Exercise professional autonomy and make critical judgements in their field of expertise. Monitor and record clinical outcomes through evaluation, audit and review. Ensure evidence-based practice in all clinical work and promote the development of an evidence base in area of specialist expertise. Order specific diagnostic interventions.

**Leadership and Management**: Lead and consult on the development, implementation, monitoring and evaluation of policies/guidelines/protocols relevant to specialist area and wider health care arena. Lead within clinical governance processes, including adverse incident reporting/investigation and management of complaints, whether relevant to their area of practice/expertise or outside. Take a lead role in preparation for Peer Review programmes Take an active lead role for audit national and local audits disseminating any relevant information resulting from the audits that will help improve the diabetes care delivered in the trust. Act as a role model, to promote consistently high standards of professional practice.

Ensure that nursing care is based on sound evidence and rationale within the departments, linking with other members of the wider team where appropriate. Work across professional, organisational and system boundaries proactively developing and sustaining new partnerships and networks to influence and improve health outcomes and healthcare delivery systems. Develop strategies to regularly review patient and user perceptions of specialist area to facilitate the engagement of patients and carers in service development. Collaborate with colleagues who lead service and business planning with regard to cost pressure and prioritising.

Manage and be an authorized signatory for relevant hospital and charitable fund accounts. Line manager and provide clinical supervision as appropriate to the role. Participate in the on-call rota. Clinically deputise for the Senior Clinical Lead in their absence.

**Education & Training**: Participate in provision of specialist education and training programmes for health care professionals and patients. Utilise professional knowledge and expertise in education, training and development in the field of expertise. Identify and respond to learning needs of individuals and teams to develop advanced knowledge and skills. Take the lead role in the professional development of the staff through active planning of the formal courses each staff member undertakes at a given time without compromising the service delivery at any time of the course.

Support the integration of theory and practice through formal and informal methods, including clinical educational opportunities. Maintain relationships with university partners to support the aims of the profession and the service, including contribution to formal and informal curriculum activity. Support colleagues undertaking higher level and postgraduate study by acting as mentor or supervisor. Improving Quality and Developing Practice: Demonstrate a commitment to evidence-based practice and clinical excellence and lead staff in the implementation of such practice.

Support and facilitate practice development, audit and project/research activity within the diabetes service, contributing to the continuous improvement in service quality. Raise the profile of the service at local, national and international level by contributing to professional and service debate and through professional activity, publications and presentations at conferences. Use professional judgement in managing complex and unpredictable care events and uses reflective practice to learn from these experiences and in turn improve patient care and service delivery. Provide expert professional input to appropriate health improvement programmes to meet the local agenda in line with the national framework.
