**Job Summary**

The post holder is responsible for supporting the smooth running of the of the Practice, and helping our patients to access and navigate services. To work alongside and support the reception team, the Medical Secretary, Clinical Staff, and the Practice Manager.

**Principal Duties and areas of Responsibility**

Support of reception administration
- Dealing with SystmOne (our clinical notes system) Tasks
- Non-clinical triage of Online Consultations
- Support for reception team on busy mornings and cover during holidays and sickness

Support for Nursing team
- Supporting frailty work and visits
- Supporting practice in coordination of housebound patient vaccinations
- Supporting health checks - Learning Disability and Serious Mental Illness care planning

Support for Doctors
- Collection, calculation and dissemination of Blood Pressure reports to GPs
- Audit appointments that have been booked by patients online for appropriateness

General support to Practice
- Administration of the carers register, including support to staff to identify carers
- Signposting to appropriate services
- Supporting practice recall processes
- Support Practice Manager with engagement with Patient Participation Group to ensure meetings are regular, including increasing group membership.
- Assisting patients and their carers by answering queries which allow them to navigate or signpost their own health needs
- Promote health campaigns, including waiting room posters and leaflets
- Promoting NHS app and supporting practice with access to records
- Supporting patients who require interpreters and arranging review and appointments as appropriate
- Develop and improve practice communications with and to patients about any changes and general information.

**Additional duties**:

- Scanning documents onto SystmOne and forwarding these to the appropriate clinician
- Dealing with Medical Report requests and typing these to send back
- Holiday cover for administrative colleagues when she/he is on annual leave
- Maintaining a tidy working environment at all times.

**Other Responsibilities within the Organisation**
- Awareness of, and compliance with, all relevant practice policies/guidelines,
- A commitment to life-long learning and audit to ensure evidence-based best practice
- Contributing to evaluation/audit and clinical standard-setting within the organisation
- Attending training and events organised by the Practice or other agencies, where appropriate.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data

- Participation in an annual individual appraisal, including taking responsibility for maintaining a record of own personal and/or professional development
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
