Social Prescriber

An exciting opportunity has arisen to join Poole North Primary Care Network (PCN) a friendly and supportive group of four GP practices working together to deliver high quality care to our 52,000 patients.

We are looking for Social Prescribers to form a new team helping to empower patients to take control of their non-medical needs, helping them focus on what matters to them and helping them connect with people and community groups.

The role will look at the population health management needs of our patients to target cohorts of those most at need to proactively deliver Social Prescribing to them.

Social prescribing can help communities, patients and medical practitioners to reduce health inequalities and can increase a person's involvement with their community and can help those with long term conditions, people who are lonely or isolated or have complex social needs which affect their wellbeing.
