Provide a specialist support service to patients, who are often experiencing complex social circumstances.

Negotiate and support access for patients to community-based services and activities that will support them to live well.

Build effective pathways to support patients to navigate existing complex circumstances.

Maintain accurate records to allow effective monitoring and evaluation of interventions.

Work autonomously to manage work load and deliver reports in an agreed format timeously.

Enable and support primary care staff to build and maintain a comprehensive knowledge of local community services and activities which can be utilised to the best interest of patients.

To undertake any other duties as deemed appropriate from time to time within the needs of the Council of Voluntary Organisations (East Ayrshire) Limited.

Must be prepared to work occasional evenings or weekends

**Job Type**: Permanent

**Salary**: £24,700.00 per year
