**Primary Mental Health Practitioner B6: Launceston and Tamar Valley Primary Care Network**

***Working in partnership with Cornwall’s Adult Mental Health Services, an exciting opportunity has arisen to join our Primary Care Network to develop and manage a Health and Well-being team.

**Main duties of the job**:
These new Primary Mental Health Practitioner roles are vital to increasing accessibility to mental health care. As the roles are jointly funded this gives the unique opportunity of working in Primary Care, with GPs and our Mental Health GP Lead, to develop a mental health and well-being team. There is reassurance and support provided by mental health colleagues within Cornwall’ Adult Mental Health Service, providing access to specialist mental health supervision and training. There is also a developing network of Primary Mental Health Practitioners across Cornwall, including a growing Communities of Practice neighbourhood.

The aims of these roles are:

- To provide rapid and responsive assessment, signposting and brief treatment to individuals referred to the service by several routes including self-referrals and General Practitioners.
- To provide support to Primary Care staff in the local management of patients with common mental health problems.
- To offer short term mental health interventions to support this client group within primary care settings, working at the primary secondary care interface to facilitate movement of clients between primary and secondary care services

The Primary Mental Health Practitioner will work with the Primary Mental Health Clinical lead to deliver high quality mental health care within the PCN and contribute to developing the service.
