JOB TITLE: Senior Facilitator - Neighbourhood Working

Contract: Fixed Term Contract - 8 months

Hours: 37 per week

Salary: £27,852 - £29,439

An exciting opportunity has arisen for a Senior Facilitator to work with us in our Community Care services. This is a new role to Rutland and the post holder will work closely with Primary Care, Rutland County Council and Voluntary Sector colleagues. If you have experience of working in a health or social care environment, can provide data, coordinate, support clinicians and provide robust administrative support we want to hear from you.

About the role

The Senior Facilitator will undertake the following:

- To co-ordinate the care and multi-disciplinary involvement of those people being managed under the Integrated Neighbourhood.
- Take a proactive approach to using population health management data held on record to identify patients who would benefit from proactive preventative support
- Support the identification of voluntary sector support and maintain, promote, and coordinate the social prescribing platform
- To act as a central point of information and a guide to processes for both health and social care services in the neighbourhood to improve working practices

About you
- NVQ3/QCF in Social care or equivalent relevant qualification
- Relevant experience of working within a health, social care or voluntary sector setting
- Administrative experience related to service users or patients
- Awareness of the needs of vulnerable people, including older people
- Can demonstrate a systematic approach to prioritisation of work and cope under pressure to meet deadlines
- Able to communicate with others in a positive and respectful manner

The Senior Facilitator is based within the defined neighbourhood of Rutland, to improve joined up working practices leading to more efficient patient/service user care. As part of this role, the post holder will take on the administrative management of the regular multi-disciplinary team meetings and agreed actions from these meetings. The Senior Facilitator will work with all health and social care professionals, as well as statutory/non statutory agencies, to provide a seamless, integrated service to our service users.
