**Job Summary**:
The post holder will have responsibility for the management and supervision of both the Senior Mental Health Practitioners managers and wider Mental Health Practitioner staff group within the Primary and Community Mental Health Transformation Programme. The post holder will provide strong leadership to the team to ensure it performs in the most efficient and effective way ensuring all key performance indicators are met whilst providing a safe and high quality service. They will manage a team of staff, help develop/maintain service policies and may also carry a clinical caseload. When managing staff, the post holder will be responsible for providing clinical advice, monitoring targets, making sure staff are adhering to service policies and supporting staff’s wellbeing and development. Where a clinical caseload is maintained the post holder will adhere to the professional standards outlined in this job description and in line with General Practice and Primary Care Sheffield policies and procedures. The post holder will take a lead in wider service development and maintain professional and positive working relationships with stakeholders and partner organisations.

**Key Result Areas**:

- Management of a small patient group in line with local General Practice procedures and risk management protocols.
- Responsibly for the provision of the city wide On call duty for PCMH
- Supervision and support of other team members.
- Participation in service development and clinical governance structures.
- Provide direct service management and clinical supervision for a designated cohort of staff, providing leadership and effectively contributing to service development
- Provision of leadership to the Team in respect of psycho-social interventions to people and carers.
- To work in collaboration with the senior management team and clinical leads to ensure effective service delivery and development within national guidelines and wider requirements.

**Professional**:

- Assesses needs and identifies problems relevant to the care of people referred to age of 18 and above.
- Devises a plan of care in partnership with the person and carers.
- Implements the planned programme of care to ensure a high standard is achieved.
- Reviews the effectiveness of the care provided and where the appropriate initiates any action.
- Advises and supports the person and carers to promote health and well-being and to prevent illness.
- Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
- Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.
- Functions as a member of a multi-disciplinary team.
- Management of enquiries and direct referrals of people in accordance with the Practice.
- Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Managerial:

- To provide strong leadership and vision to the team maintaining robust quality assurance to enable a safe high quality service to be delivered.
- To direct staff and provide expert advice and guidance on the management of patient care.
- To ensure that work allocated to staff is commensurate with their skill set and level of training.
- To manage and maintain systems for planning and allocation of work and for the setting and reviewing of targets, standards and priorities to the team.
- To ensure professional supervision systems are in place.
- To participate in the recruitment and appointment of staff within agreed HR procedures.
- To hold regular team meetings and develop the Mental Health Practitioner staff group.
- To respond to enquiries and complaints within the framework of the complaints procedure.
- To work in partnership with other statutory, independent sector and other agencies, in the effective discharge of the duties of the post and as part of wider service development.
- To manage staff sickness through the agreed sickness management policy/ procedures and to provide regular monitoring reports as required.
- To deal appropriately with clinical incidents and complaints in accordance with General Practice and Primary Care Sheffield policies.
- Ensure all staff are appropriately trained to meet the requirements of their role and any issues with performance are addressed, explored and managed.
- To work with and monitor performance indicators to achieve key performance indicators.
- To ensure all resources are used effectively and efficiently and within budget.
- To contribute to audits as required, providing relevant data as required in line with performance monitoring targets.

**Administrative**:

- Maintains accurate patient care records in accordance with professional codes of conduct and Practice procedures and ensures records are made available when required by the appropriate agency.
