**Job overview**:
This is an exciting opportunity to work as part of a service delivering reablement in peoples own homes as part of the Kerrier Home First service which is a collaborative approach between community, acute and social care providers. Consideration for full and part time workers.

You will be based in either Camborne & Redruth or Helston community hospital depending on your preference.

You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.

You will be part of a dedicated team and will be provided with bespoke training to support your development in this role.

You will be required to work a rota that covers 7 days a week from 08:00 to 20:00. Flexibility within this rota, and geographical location, will be important.

You must have a valid driving licence. This is a lone worker post.

**Main duties of the job**:

- The main purpose for this post is to ensure that opportunities for patients’ reablement are optimised within a community setting.
- The post holder will be rostered with other Community Support Workers to provide a 7 day a week service to community patients requiring reablement either before or after a hospital admission.
- The Reablement workers within each locality will form a part of an Integrated Care Pathway for frail patients that involve a range of services.
- This post holder will be expected to work with patients in the community, i.e. Care home setting and in patients own homes.
