Social Prescribers signpost people to community-based activities and support to improve health and wellbeing and reduce social isolation.

The postholder will offer direct support to people with low/moderate health and social care needs to access community-based opportunities as well as providing front line workers such as Social Workers, District Nurses, Occupational Therapists, Mental Health Nurses and GPs with information on community-based support and activity opportunities in the area, so they can identify people they are working with who may benefit from signposting to these.

The role will help to increase the number of people accessing community-based supports in Perth and Kinross.

This will help improve outcomes for people, especially those who do not meet the eligibility criteria for statutory supports.

**The Individual**:
Effective time management is essential for a successful autonomous worker to manage Social Prescribing duties, with the ability to be flexible and respond to competing demands being integral to the role.

This is an interesting post that will require you to demonstrate flexibility, knowledge and skills to respond to the changing needs of individuals.

Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines.

You must be able to work effectively within a team, as well as individually, and demonstrate a commitment to improving service provision whilst raising awareness of available resources and supports.

You will receive self-referrals, as well as referrals from frontline staff and partner agencies, and co-ordinate and maintain networks and engagement.

You will work well with service users and carers to promote community-based services and other supports as appropriate.
